Test Sample Collection Process:

1. Sample Collection: Retrieve your sample and carefully place it inside the mailing pouch provided in your kit.
2. Mailing the Sample: Insert the sample into the designated return envelope and ensure it is securely sealed. Drop the envelope in the mail for sending.
3. Receiving Results: Expect to receive your personalized results within a timeframe of 2-4 weeks.
4. Diet Modification: Utilize the recommendations provided as well as the guidance of your healthcare provider to make necessary adjustments to your diet.

User Instructions:

1. Open the small plastic bag and remove all components. Locate the collection card marked with a barcode on the front.
2. Visit omegaquant.com/start and register your test kit using the barcode found on the collection card. Failure to register will result in non-receipt of your results.
3. Complete the requested information on the collection card.
4. Enhance blood flow by washing your hands in warm water and standing up. Shake your hand at your side to prepare for the fingerprick.
5. Use the alcohol wipe to clean the tip of your selected finger (index, middle, or ring). Allow the alcohol to naturally dry.
6. Remove the blue protective cover from the lancet. Gently place the lancet on the clean finger and press firmly downward until a click sound is heard.
7. Use gauze to wipe away the initial appearance of blood.
8. Apply gentle pressure to the finger to encourage the formation of a blood drop. Be patient, as this may take 15-30 seconds.
9. Allow the blood drop to fall onto the collection card, specifically into the center circle.
10. Once the blood spot is collected, use the gauze pad to apply pressure on the finger and bandage the site of the prick.
11. Leave the collection card open with the flap exposed for several minutes to allow the blood spot to dry. Afterwards, fold the flap over to cover the dried blood spot.
12. Place the sample collection card inside the provided plastic bag and securely seal it. Insert the bag into the pre-paid return mail envelope and mail it on the same day.
13. Dispose of the lancet and other supplies by placing them in the appropriate trash container.
